head of the queen/king -- the deputy and the leader's personal adviser. can hold meetings, host training sessions, invite allies, lead raids, host weekly tasks and hold private meetings.
head medic -- the person in charge of taking care of the sick. can hold training sessions for those interested in medicine.
lady/lord -- assistant deputies. can lead raids and invite allies to events.

it's up to you what kind of things you put in your form, though you must include the character's name, gender, and other basic information, what rank you want, and how active you are. it mostly depends on your activity if you'll get the position.
